When sugars and carbohydrates are consumed, oral bacteria convert them into acids that can erode enamel. Over time, repeated exposure leads to cavities and decay. Fluoride acts as a natural defense system, shielding the enamel and reducing the risk of both childhood and adult dental disease—particularly beneficial for individuals with reduced saliva flow or those taking certain medications.

Used consistently as part of a comprehensive oral care routine—daily brushing and flossing, mindful nutrition, and regular professional cleanings—fluoride significantly enhances long-term dental health. While fluoride is one of the most effective elements in strengthening teeth and preventing decay, appropriate use during early childhood is essential to ensure safety and balance. Because fluoride is commonly present in community water sources and most toothpastes, monitoring its intake helps protect developing teeth from excessive exposure.

For children under the age of two, brushing should be done with an ADA-approved, non-fluoridated toothpaste to avoid accidental ingestion. Between the ages of two and five, a pea-sized amount of fluoridated toothpaste should be used twice daily on a clean toothbrush. Young children should be gently encouraged to spit out any excess foam or paste after brushing—an important habit that often requires patience, guidance, and positive reinforcement.

The amount of fluoride ingested between ages one and four can influence the development of a condition known as dental fluorosis, which may appear later as faint white specks or streaks on the permanent teeth. Although fluorosis is primarily a cosmetic concern and not harmful to health, it underscores the importance of appropriate fluoride use during early childhood.

By age eight, the risk for fluorosis diminishes significantly; however, continued use of ADA-approved fluoridated toothpaste remains essential for maintaining strong enamel and preventing decay.

The pediatric dentist is the best person to decide whether a child needs fluoride supplements.  First, the dentist will ask questions in order to determine how much fluoride the child is currently receiving, gain a general health history, and evaluate the sugar content in the child’s diet.  If a child is not receiving enough fluoride and is determined to be at high-risk for tooth decay, an at-home fluoride supplement might be recommended.

Topical fluoride can also be applied to the tooth enamel quickly and painlessly during a regular office visit.  There are many convenient forms of topical fluoride, including foam, liquids, varnishes, and gels.  Depending on the age of the child and their willingness to cooperate, topical fluoride can either be held on the teeth for several minutes in specialized trays or painted on with a brush.
